It's that time for my 86.5. Five years and 6,000ish miles.

Is it *really* worth it to do a full refresh with so few miles?

Otherwise, what are the biggest things to check for?

No.

Belt tracking on cam gears.
Belt dust, belt cracking, condition: manufacturer’s lettering on belt should be very readable and almost new looking.
Condition of cam gears.
Idler roller should look yellow-zinc plated, not black from belt slapping.
Change in belt tension since last check; tension change should be minimal since post-installation check.
Use small inspection mirror: check tracking on tension roller, should be centered.
Pull right cover, check tracking on pump pulley. If against leading edge ‘lip’ of pulley, check for occlusion of weep hole by pulley with small inspection mirror.
Check oil level in tensioner: should ‘take’ no more than a teaspoon of oil.
